Facility Fees

I currently have Medipak Advantage (PFFS) for my Medicare plan. Per policy my co-pay per office visit is $25.00.
My doctor is located in the outpatient building (it's name) of a hospital. I've seen the doctor twice and each visit I paid my $25.00 copay. I'm now being billed an additional $25.00 per visit co-pay for the facility fee. When i asked why I owe more money I was told the hospital bills separately for the facility fee. Has anyone else had this issue?

Yes I have heard of them doing this. I had a client that had a follow for a surgery that took 2 min and the doctor told he would not charge a copay for that follow up. Client got a bill because they told him the Dr didn't charge a copay so they charged him a copay for the room.
