Yes, it is possible even after meeting the Part B deductible. The anesthesiologist may not accept Medicare assignment and balance bill you an additional 15% above their Medicare allowable.

If you have surgery and are in pain while in recovery, you may receive prescription strength pain medication. Part B does not cover this type of outpatient hospital drug and the hospital pharmacy that dispenses the drug is unlikely to be in a Part D plan's pharmacy network.
 
When I go to the ER at the local hospital will I ever have to pay more than 20%?
Yes, If you do not have a medicare supplement plan then you are responsible for the Part A Benefit Period deductible which for (2018) is $1,340.00 plus 20% of the Bill above and Beyond that. Hope this Helps.

The Part A Benefit Period deductible applies to inpatient hospital services. Medicare considers the ER to be outpatient, which falls under Part B cost sharing.
