Im Confused...

I'm in a nursing home/therapy. I had surgery on my heel the first of the year and I ran out of part a. My doctor extended my therapy time. Is it possible to get on part a again without having anything done as for hospital time.

Medicare Part A (Hospital Insurance) may cover care given in a certified skilled nursing facility (SNF) if it's medically necessary for you to have skilled nursing care (like changing sterile dressings). However, most nursing home care is custodial care, like help with bathing or dressing. Medicare doesn't cover custodial care if that's the only care you need.

In another words, you are not entitled for nursing home care, unless you are sent to a skilled nursing facility and only when admitted to the hospital for a minimum of 3 days. You said you exhausted Part A, did that include your lifetime reserve days?
 
Part A is based on Benefit periods. Your benefit period ends after you have been out of the skilled nursing facility for 60 days in a row. So if it has been longer then 60 days then yes your part A coverage would reset and you would have another 100 days for skilled nursing facility
