Medicare deductible Part A

My brother went directly from the hospital to a rehabilitation facility. He was charged a $1000 deductible for EACH one (total $2000).
Should he have been charged just once?

Does he have Medicare Part A and Part B?

Does he have a Medicare Supplement Plan, or Medicare Advantage? If so which one, and who is the insurer, and which plan does he have with the insurer?

Any other information you can provide will help someone to answer if possible.

He has parts A and B.
No Supplement Plan or Advantage.

Medicare Part A covers hospital room and board with a deductible of $1,184 for up to a 60 day stay. This does not include anything done to him while he is in there which would be considered under part B. Part A also covers skilled nursing facilities inpatient after a three night stay in the hospital. The first 20 days are covered at no charge. If he went to a skilled nursing facility after a three nights stay in the hospital, than the SNF should not be charging anything until day 21.
