The way the Medicare payment system is set up makes it fertile ground for those who are less than honest. There are currently over 18,000 diagnosis codes that providers use to bill Medicare and Medicaid and that system will expand to over 140,000 codes next year. It is easy for fraudsters to file a claim using payment codes that pay higher fees than the code of the procedure actually performed.
With over 4.5 million claims processed a day, and over 100 million people covered by Medicare and Medicaid, it is little wonder that about 80 billion dollars in fraudulent claims are paid out each year.
